Health Services:  The following is a list of state recommendations of health services that will be provided by your school nurse .

• HEARING SCREENING-GRADES KINDERGARTEN THROUGH  3rd GRADE
• VISION SCREENING-GRADES KINDERGARTEN, 2ND GRADE, & 4TH GRADE
• HEIGHT, WEIGHT AND BLOOD PRESSURE -ALL GRADES
• SCOLIOSIS 5TH GRADE
• PHYSICALS ARE REQUIRED TO ENTER PUBLIC SCHOOL AND ARE ALSO RECOMMENDED TO ENTER 6TH GRADE AND 11TH GRADE

If you would like your child to be excused from any of the above, please provide a letter to the attention of your school nurse.  These regulations were generated in the best interest of your child's health.   Let us continue to work together to maintain the health and safety of our children.

Physical Examinations: A complete physical is required for all students upon initial entry into the school district. Your health care provider must conduct the examination, since he/she is more familiar with your child's health history and is in a better position to recommend immediate steps for any remedial care. The physical findings will need to be recorded on the district-approved form, which will be provided for you.  The school doctor will provide an examination during the school year for students that do not have a healthcare provider
