2021-22 Sandshore Kindergarten Registration

All eligible kindergarten students must be 5 years old on or before 10/1/2021. →

(All eligible first grade students must be 6 years old on or before 10/1/2021.) →

Pre-register: Click here to register your child for the upcoming 2021-2022 school year. If a virtual meeting is needed, you will be contacted by the registrar's office.

Complete health forms and transportation request form.

Universal child health record form, with immunization records in English attached, signed and stamped by a physician.

*The State of New Jersey requires that the physical examination must be completed within 365 days of school entrance. Submission of this form is mandatory to attend school.

Dental form: Must be signed by a dentist.

Vision form: Must be signed by a doctor.

Annual medical update form.

Transportation request form.

Original birth certificate of your child with raised seal (a copy is not acceptable).

Proof of residency. The residency proof MUST be in the parent's name.

FOR HOMEOWNERS: your deed, mortgage statement or property tax assessment, and one of the following: utility bill, driver's license, voter registration, permits, or other expenditures demonstrating personal attachment to a particular address.

FOR RENTERS: your lease and one of the following: utility bill, driver's license, voter registration, or other expenditures demonstrating personal attachment to a particular address.

MMR (Measles, Mumps, Rubella)

Varicella  (Chickenpox)

Hepatitis B

A total of 4 doses with one of these doses on or after the 4th birthday OR any 5 doses.

A total of 3 doses with one of these doses given on or after the 4th birthday OR any 4 doses.

Measles: 2 doses of the Measles vaccine. First dose on/after 1st birthday.

Mumps: 1 dose of the measles vaccine needed.

Rubella- 1 dose of the Rubella vaccine needed.

1 dose on or after first birthday.

Or

Laboratory evidence of immunity, physician's statement or a parental statement of previous varicella disease is acceptable.

3 doses of the Hepatitis vaccine are needed.
